Serious Illness Services (Palliative Care)

Palliative care for chronic illnesses improves the quality of life for you or your loved one

At Mountain Valley, we understand that every moment matters. That’s why our palliative care and serious illness services are designed to help you or your loved one make the most of each minute, enhancing the quality of life.

We offer palliative care to people of all ages from infancy and onward, who are experiencing  a serious, life-limiting illness. We provide specialized medical support that concentrates on symptom control and comfort-focused care.

Serious Illness Services (Palliative Care)

Unlike hospice care, which begins when the patient has six months or less to live, our palliative care practice offers medical care when someone has a serious illness and a prognosis of 24 months or less if the disease were to continue to progress.

A palliative care patient can go into remission or even recover and no longer require palliative care. Some patients may continue in palliative care and then transition into hospice care. Palliative care offers an added layer of support, working alongside the patient’s primary care doctor and other medical specialists.

Patients who are seen by a palliative care specialist continue to see their other doctors and will often continue to seek aggressive care such as surgeries, chemotherapy, blood transfusions, organ transplant, etc.   Specialized medical support from a palliative care provider you can trust.

Mountain Valley’s serious illness services can minimize ER/hospital visits and reduce hospital admissions. Our team works with the patient’s healthcare team, providing an additional level of support and care. We serve both adult and pediatric patients.

One of the key benefits of palliative care for chronic illnesses is symptom management. We can help patients with disease-related symptoms such as:

  • Pain
  • Dyspnea (shortness of breath)
  • Nausea
  • Vomiting
  • Constipation
  • Anxiety
  • Agitation
  • Sleep disturbance

We can also discuss advanced care planning, providing crucial information you need to make educated choices.

Do you qualify for our palliative care/serious illness services?

  • COPD or lung diseases
  • Heart failure or heart diseases

What is the difference between hospice vs. palliative care?

We understand that you may have some questions about hospice vs. palliative care. The following chart may help you.

Patient Prognosis

Hospice

Life expectancy of 6 months or less.

Palliative Care

Qualification for palliative care does not require a prognosis of 6 months or less. Patients can continue to receive curative treatments while receiving palliative care. Some people who are eligible for hospice but not psychologically ready for hospice will seek support from palliative care.
