We manage end-stage lung disease symptoms–enabling you to receive help at home
Those with end-stage lung disease frequently visit their doctor’s office or ER due to symptoms such as fatigue and difficulty breathing. But with hospice in the home, we can manage and minimize the effects of these symptoms. In some cases we can even prevent them.

We’ll let you know if you qualify for the program
It’s very simple to determine if you’re a candidate for our Advance Lung Care Program. In general, all hospice patients with advanced lung disease qualify.
Our providers and nurses will work closely together, examining medical history to ensure that you meet the outlined criteria. Once it is determined that you are eligible, your nurse will provide all the information and tools you need.
With our Advanced Lung Care Program, you can expect several benefits
Some of the advantages of our program include:
- You’ll receive regular visits from one of our specially-trained nurses to evaluate your condition and to help effectively manage this disease through hospice in the home. This nurse will work under the orders of your physician or care team.
- Your family will receive a wealth of knowledge through our patient education materials, and our care team is always available to answer any questions.
- Your Advanced Lung Care Program team will be overseen by our board-certified Hospice and Palliative Medicine physician. They will work directly with your healthcare team, including your primary care doctor.
- Your physician will order medications designed to control your symptoms. Our nurse will review these with you and order those that are essential.
- We recognize that hospice at home involves much more than physical health. You’ll have easy access to visits from a social worker, a nurse assistant, a chaplain, and counselor or any other member of your care team that would be helpful.
